Year 10

Universal

  • Look at websites for potential settings.
  • Explore the Help You Choose website which contains all post-16 courses in Norfolk as well as information on voluntary organisations, apprenticeship and traineeship vacancies and training for work opportunities.
  • Signpost families to the FYI transition resources.

Targeted

Enhanced

  • Discuss potential Post 16 settings with SENDCo and EHCP Co at Y10 annual review.
  • Arrange a meeting or visit potential setting to discuss support and gather information relating to Post 16 provision.

Year 11 Autumn term

Universal

Targeted

  • Visit potential settings.
  • Ask for a meeting with potential setting to gather an overview of support available.
  • Upload INDES and other SEND documents to Help You Choose application form for 1st choice post 16 setting.

Enhanced

  • Arrange visit with Post 16 setting.
  • Complete the Aspiration Tree Document (part of annual review template) as part of the annual review.

Year 11 Spring term

Universal

  • Post-16 settings start their interview process. Make sure the post-16 setting is informed of any additional needs at this stage.

Year 11 Summer term

Universal

  • If the results are not as expected, contact post-16 setting who will offer support and can switch courses as appropriate.

Targeted

  • Ensure that all key information and routine arrangements are known, such as the location of different areas of the campus, name of key worker, names of teachers/lecturers/tutors.

Year arrived in new setting Autumn term

Universal

  • Attend all induction offers.
  • Start looking at future steps and explore websites such as UCAS, or apprenticeship websites.

Year 10

Universal

  • Arrange appointments with Independent Careers Advisor for young people.
  • Invite Post 16 settings to CIAEG events and / or family facing events.

Targeted

  • Collate list of young people with EHCPs and Post 16 options. Start preparation for adulthood interventions linked to outcomes set in Y9.

Enhanced

  • Encourage families to attend Open Evenings and speak to the prospective Post 16 settings to allow young people and their families to make informed decisions.
  • Hold annual review meeting, discuss possible options for Post 16 provision with EHCP co and Independent Careers adviser.

Year 11 Autumn term

Universal

  • Signpost families to Open Evening / events at Post 16 settings.

Targeted

  • Make a referral to TITAN travel training for young people who would benefit from independent travel training.
  • Ensure UK school systems / processes around applying for school places and transition are explained as soon as possible to families who are newly arrived in the UK or who are not proficient in English.

Enhanced

  • Hold EHCP review to ensure EHCP is fit for Post 16 setting and outcomes are adapted accordingly.

Year 11 Spring term

Universal

  • Ensure that everyone has applied for Post-16 settings.
  • Post-16 settings start their interview process. Make sure the post-16 setting is informed of any additional needs at this stage.

Targeted

  • Provide the post-16 settings with details of young people requiring targeted or enhanced transition processes with consent from the children and young people and their families.
  • For young people applying for apprenticeships and traineeships, support the young person to keep checking current openings.
  • Consider whether it would be useful for the post-16 setting to observe young person in lessons, in order to see how they access learning.

Enhanced

  • Consider whether it would be useful for the post-16 setting to observe young people in lessons to see how they access learning.
  • Begin to implement transition plans.

Year 11 Summer term

Targeted

  • When the young person is offered a place, firm up any preliminary discussions with the post-16 setting about any additional support needed and or the arrangements for additional visits.
  • Use the 'Help You Choose SEND Document upload' function for secure transfer of information. Careers Adviser can upload information after consultation with SENDCo. Ensure permission / consent is obtained from young person and family.

Year arrived in new setting Autumn term

Universal

  • Ensure all data and files are transferred securely to confirmed Post 16 setting.

  • Welcome families and young people to open evenings, events, and informal meetings.

Enhanced

  • Respond to consultations. Clearly outline any provisions from Section F that the setting will need support in implementing.

  • Hold discussions with post-16 settings about individuals requiring support with enhanced SEND provision.

Year 11 Autumn term

  • Host information events for young people with SEND and their families.
  • Collate lists of young people who may potentially need targeted or enhanced transition support for future reference.

  • Provide information on taster events and any summer transition events for new prospective young people to join.

Targeted

  • Hold meetings with families of young people with SEND to gather information.
  • Request information from feeder setting for all young people and any additional information for young people with SEND.
  • Ensure any communication barriers the family or young people have e.g. EAL are identified in collaboration with the current setting and families e.g. through admissions forms.
  • Ensure families and young people are provided with information in an accessible format (language).

Enhanced

  • Begin any enhanced transition programme for young people with high needs.
  • Arrange initial planning meeting with families and young people.
  • For EHCPs named at setting arrange initial planning meeting with families and young people.
  • Design and deliver a transition plan for young people with EHCPs.

Year 11 Summer term

Universal

  • Host welcome / transition events for incoming cohort.

Targeted

  • Ensure that all young people with SEND are aware of and offered a place on their induction programme.
  • Follow up with a communication to new young people re-emphasising next steps and what happens.
  • Download SEND documents from Help You Choose.

Enhanced

  • Contact the The SEND and inclusion team for any support required with provision mapping and support, this could also include support for young people with EAL or who are GRT.
  • Discuss and plan implementation of Section F provisions with family and incorporate into transition SEN Support Plan.
  • Invite young people to any enhanced transition activities.

Year arrived in new setting Autumn term

Universal

  • Welcome young people to activities such as induction and share information on how to swap courses if required.
  • Provide information about Post 16 services and how to access them e.g. bursary/bus passes.

Targeted

  • Provide information about SEND services available to young people including any additional provision that can be accessed.
  • Support young people who require additional support through targeted transition and induction activities.

Enhanced

  • Implement support packages to those young people who need this e.g. timetable with additional support sessions.
  • Meet with young person and family to check support package is working as it should be.
  • Arrange appointments with Careers Advisor for young person.
