Introduction to transition timelines

The aim of these timelines is to provide schools and settings with a suggested timeline to complete phase transfer activities for children and young people at statutory school age to provide clear, cohesive and welcoming transition experiences across settings.

There are four timelines: EYFS, Y2-Y3, Y6-Y7 and Y11-Post 16. Each individual timeline is organised into terms and highlights three different area of focus:

  • Families and children and young people
  • Current setting
  • Receiving setting

The transition support outlined is also organised into three levels: Universal, Targeted and Enhanced.

  • Universal includes transition provision for all children and young people.
  • Targeted includes universal provision as well as additional transition provision for some children and young people which may need a slightly higher level of support such as those who are GRT, EAL, on SEND support plans or those who are young carers.
  • Enhanced includes universal and targeted provision as well as additional transition provision for a few children and young people who need a highly personalised level of support such as those who have an EHCP or a more complex combination of needs.
We recognise that schools and settings have their own systems and processes around transition, the activities and timings within this timeline are suggested. Make sure you plan activities and events at times that work best for your children and young people, your families and your school calendar.
