Year 10

Universal

• Look at websites for potential settings.
• Explore the Help You Choose website which contains all post-16 courses in Norfolk as well as information on voluntary organisations, apprenticeship and traineeship vacancies and training for work opportunities.
• Signpost families to the FYI transition resources.

Targeted

• Review SEN Information Reports and the SEND section of the Help You Choose website.
• Have a look at Preparing for adult life section of the local offer.

Enhanced

• Discuss potential Post 16 settings with SENDCo and EHCP Co at Y10 annual review.
• Arrange a meeting or visit potential setting to discuss support and gather information relating to Post 16 provision.

Year 11 Autumn term

Universal

• Attend Open Evening / Open Day events.
• Create an account on Help You Choose.
• Make applications to Post 16 setting via Help you Choose or provider websites.

Targeted

• Visit potential settings.
• Ask for a meeting with potential setting to gather an overview of support available.
• Upload INDES and other SEND documents to Help You Choose application form for 1st choice post 16 setting.

Enhanced

• Arrange visit with Post 16 setting.
• Complete the Aspiration Tree Document (part of annual review template) as part of the annual review.

Year 11 Spring term

Universal

• Post-16 settings start their interview process. Make sure the post-16 setting is informed of any additional needs at this stage.

Year 11 Summer term

Universal

• If the results are not as expected, contact post-16 setting who will offer support and can switch courses as appropriate.

Targeted

• Ensure that all key information and routine arrangements are known, such as the location of different areas of the campus, name of key worker, names of teachers/lecturers/tutors.

Year arrived in new setting Autumn term

Universal

• Attend all induction offers.
• Start looking at future steps and explore websites such as UCAS, or apprenticeship websites.
